Please have Safety Sheets, Driver Card and Handler QMA Badge or Gold copy with you when you come to registration. If both parents will not be attending please print the two sided Parent/Minor Waiver Form, sign, have notarized, and bring it with you. If both will be attending, the forms will be available at registration. (This is a QMA Rule)
